In an attempt to try to see if ntfs_remount() was being called reliably or not, I tried calling it at various times in the boot after the failed mount to see if the machine would boot. It appears that on some DC models, the boot does not go through the ntfs_remount() at all and seems to halt on the failed mount. This leads me to believe that I am doing something wrong in the NTFS calls that causes subsequent boots to fail me. I will have to research this further.
In the case of ntfs_remount() it seems to be getting called for both controllers - that is, it is called only once per boot. In the case of the fail to mount, it is only called for the OS drive. So, something is wrong with my ntfs_remount() function.
I have a custom IDE-SCSI SATA controller that runs two PCI-Express SCSI ports, each of which is a (2x) SATA port (via a VIA VT6282) on an x8 slot. The x8 slot is on the motherboard, not on the custom card. The two cpus have a right-angled cable connecting them. The connectors on the cabling are not backplanes. The system boots to a VM and the cabling is plugged in. As it boots, the BIOS runs the boot-flash routine and passes the bootstrap to the operating system which starts. The operating system is paused while the BIOS enumerates the SCSI devices and provides services (such as alternate boot mechanism).
